  • Unique serial or transaction number for each shredding job

  • Date and location of where the materials were destroyed

  • Name of the driver who picked up and transported all documents

  • Name of employee who witnessed the destruction and processed it for recycling

  • Name and signature of the witness to the destruction

  • Clear statement of the terms and conditions

  • Statement of fiduciary responsibility
